As a Registered Social Worker practicing in Ontario, I am committed to protecting the privacy and confidentiality of your personal health information. This Privacy Policy outlines how your information is collected, used, stored, and disclosed in accordance with Ontario’s Personal Health Information Protection Act (PHIPA).

I collect personal health information directly from you or from a person acting on your behalf. This information may include:

  • Session notes and assessments
  • Information provided through online sessions, emails, or text messages

Your personal health information is collected for the purpose of:

  • Providing you with safe and effective counselling services

  • Communicating with you about appointments, scheduling, and treatment plans
  • Meeting regulatory and legal obligations (e.g., documentation, supervision, reporting obligations)

4. Communication via Email and Text
I may communicate with you via email and text messaging for administrative purposes such as appointment reminders, scheduling, or brief check-ins. While reasonable efforts are made to ensure the security of these communications, please be aware that email and text are not guaranteed to be secure forms of communication. You may opt out of these methods at any time by notifying me.

5. Use and Disclosure of Information
Your information is kept confidential and will not be shared without your written consent, except in the following circumstances:

  • If you disclose that you or someone else is at risk of serious harm

  • If required by law (e.g., court order, child protection concerns)
  • If disclosure is required to report abuse of a vulnerable person
  • For professional supervision purposes, with identifying details removed wherever possible

6. Storage and Security
All records are stored securely in password-protected digital files or locked physical storage. Reasonable measures are taken to protect your information from loss, theft, or unauthorized access.

7. Online Counselling
Online counselling services are provided via secure video platforms that are compliant with PHIPA and PIPEDA standards. You are encouraged to attend sessions in a private space and use a secure internet connection to ensure confidentiality.

8. Access to Your Records
You have the right to access your health records and request corrections if necessary. Requests can be made in writing and will be responded to within 30 days.

9. Retention of Records
Your records will be retained for at least 10 years after your last session, or 10 years past the age of majority if you were a minor at the time of service, as required by law.
